There is air coming out the front of the case, near where the processor is ( its a dell BTX case), but when i put my hand infront of the grill on the back, air was coming from there too. It seems as if air is being sucked out from front and back ( don't know how thats possible). If i add an extra case fan on the grill, should it extract, or blow??
Just looked up that case....my sister had the same 1, before I built her a new computer when the motherboard died.
Have you ever looked inside the case? That BTX case is so jumbled that it is not conducive for excellent airflow. You caould try adding another fan, but make sure you open the case to make sure it's going to fit where you want it.
To be honest though, you should not be worried about airflow unless your temperatures are too high. You can download a program such as speedfan to monitor the temps.

I can see all that in 1 glance. Anyway, unless your cpu temperature is high 50's or low 60's Celsius, then you don't need to add another fan. If you're going to do it anyway, make sure it will fit in there before you buy it.
Oh, and if you add a fan to the front, it should be blowing air towards the rear of the case.
